The benefits of Medicare Supplement (Medigap) Plan M are the same as those of Medigap Plan D, with the exception that you must pay half of the Medicare Part A deductible with Plan M. (while Medigap Plan D covers the entire Part A deductible).

The Medicare Part B deductible and excess costs are not covered by Medigap Plan M. Excess charges under Part B are expenses that are not included in the Medicare-approved fee. For example, assume the maximum charge allowed by Medicare for a specialist appointment is $100, but the provider refuses to accept that amount and instead charges an additional 15%. In this case, Medicare will pay 80% of the allowable fee, resulting in a payment of $80 to the physician. The recipient is responsible for not only the remaining $20, but also the extra 15% tax — an additional $15, for a gross out-of-pocket expense of $35.
There is no out-of-pocket cap for Medigap Plan M, which means you are responsible for any expenses not covered by Original Medicare or Medigap Plan M. When a Medigap plan has an out-of-pocket spending cap, that ensures that if you've spent that much on out-of-pocket expenses and your annual Part B deductible, the plan will cover all covered services for the remainder of the calendar year.
